2025 Hometown Fund Application 

We highly recommend completing the Self-Assessment Tool before beginning the 2025 Hometown Fund Application. 

When you first access the Hometown Fund Application, you will be asked to answer some eligibility questions. Your answers to these questions determine if your organization meets the requirements of this grant opportunity. Once you have completed the eligibility questions, you will access the application's background and narrative form. The application's narrative form is comprised of eight (8) text responses, two (2) attestation fields, and three (3) video or text responses. 

If you choose to respond to a question via video, please refer to the additional notes in the helper text for length guidance. Please make sure that the video you upload does not exceed the requested video length. Accepted video types are .3gp, .avi, .flv, .m4v, .mkv, .mov, .mp4, .mpg, .webm, .wmv. Please note that the reviewers will focus on the content and messages conveyed in the video rather than the production quality. Therefore, please ensure that you fully answer the questions asked in the application. We also encourage you to be mindful of your audio and visual backgrounds when filming your responses. Any submitted video responses will be a part of your overall score for individual application sections. 

The review team can only access your application and video responses through the submission portal. The application scoring rubric, linked in the Resources, contains details about what reviewers will look for in each section.
