When you first access the application, you will be prompted to respond to eligibility questions. Your response to these questions determines if your system of care meets the requirements of this grant opportunity. After the eligibility questions, you will access the application form. The application form is comprised of nine (9) text responses, four (4) check boxes, and two (2) video uploads. Upload requirements include a project plan, project budget, and letters of commitment from collaboration members, if applicable.
Video response questions are labeled as “video” and include a note with the maximum video length. Please do not upload a video that exceeds the requested video length. Please note, the max file upload size is 400mb, and the maximum total submission size per application is 800mb. Accepted video types are: .3gp, .avi, .flv, .m4v, .mkv, .mov, .mp4, .mpg, .webm, .wmv. The reviewers will pay attention to the context and expressions shared during the video, not the production quality. Be sure to respond fully to the questions asked in the application. We encourage you to be mindful of your audio and visual backgrounds when filming your responses. The video responses will be a part of your comprehensive score for individual application sections.
The review team will only access your application and video responses through the submission portal. The application scoring rubric contains further information about what reviewers will look for in each section.

Please list examples of how other projects have measured the long-term outcome of “improving health outcomes of program participants with chronic conditions living within the Health Zone."
- Examples of long-term health outcomes the CVS Health Foundation has seen from successful Health Zones projects include improvements in individual health outcomes, such as BMI, blood pressure and A1c levels. Interim indicators have included measurements of reduction in ER utilization and hospital readmission, improved medication and/or testing adherence, and changes in attitudes and/or beliefs.
Can an eligible Health Zones project include a for-profit cohort partner?
- While a majority of the grant funding should support non-profit activities, a portion of the grant can be distributed from the lead agency to a for-profit partner for services that benefits program participants (examples include Uber Health and InstaCart Health).
Will the CVS Health Foundation accept multiple submissions by one organization?
- Yes, the CVS Health Foundation would be happy to review multiple submissions by one organization.
Must the people served reside in Columbus, OH?
- Through this initiative, the Foundation intends to fund projects serving residents of the city of Columbus, and preference will be given to organizations that define a patient population by under-resourced location (e.g. by specific neighborhoods, census tracts, etc.). The Foundation intends to fund projects that primarily focus on underserved patient populations (e.g., those living at or below the Federal Poverty Level, those who are under- or uninsured). Projects and partnerships that are not serving participants directly in the city of Columbus, Ohio are not eligible to apply for this grant.
